Care plans are essential documents that outline the specific care and support each client requires. In Skedulr, creating a care plan is straightforward and helps ensure your team delivers consistent, high-quality care.

Step 1: Navigate to Clients

From your main dashboard, click on 'Care Plans' in the left sidebar menu. This will take you to the care plans overview page where you can see all existing care plans.

Step 2: Create a New Care Plan

  • Click the 'Create Care Plan' button in the top right corner
  • Select the client this care plan is for from the dropdown menu
  • Enter a name for the care plan (e.g., 'Morning Care Routine')
  • Add a description outlining the purpose of this care plan

Step 3: Add Care Plan Details

Now you can specify the details of the care required:

  • Personal care needs (washing, dressing, grooming)
  • Medication administration requirements
  • Mobility assistance and positioning
  • Dietary requirements and meal preparation
  • Activities and social engagement

Pro Tip — Include specific preferences and important notes about how the client likes things done. This helps carers deliver personalized care that respects individual preferences.

Step 4: Set Care Objectives

Define clear objectives for this care plan. What outcomes are you working towards? Examples might include:

  • Maintaining independence with personal care
  • Ensuring medication compliance
  • Promoting social interaction and mental wellbeing
  • Supporting mobility and preventing falls

Step 5: Review and Save

Once you've entered all the information, review the care plan to ensure everything is accurate and complete. Click 'Save Care Plan' to finalize.

Important — Care plans should be reviewed regularly (at least every 3 months) and updated whenever there are changes to the client's needs or circumstances.

Next Steps

After creating your care plan, you can:

  • Assign the care plan to specific visits
  • Share the care plan with your care team
  • Print or export the care plan for client records
  • Track care plan compliance and outcomes
